The American Academy of Private Physicians (AAPP) is a nonprofit organization founded in 2003 for the purpose of supporting and fueling the growth of medical practices that provide “concierge” and other forms of personalized, value-based medical care.

Cancellation/Refund Policy

All cancellations must be received in writing. Any cancellation received on or before 30 days prior to the first day of the conference is eligible for a full 100% refund. All cancelations received within 30 days of the conference opening day will be subject to a $100 cancellation fee. A cancelations fee is charged to cover costs such as food and beverage that have been guaranteed as a result of your registration to the host facility. Conference “no-shows” will not be eligible for any refund. Substitutions from the same company can be made at no charge (see Substitution Policy).

Substitution Policy

A substitution of registration, from the same company, is permitted up to the opening of the AAPP Conference. Only one substitution is permitted per original registrant. The individual submitting the substitution request is responsible for all financial obligations (any balance due, including the difference in membership rates if applicable) associated with that substitution as well as updating any contact information. Substitutions cannot be made after a badge has been issued.

The Bale/Doneen Method challenges the current standard of care by proposing a disease treatment paradigm rather than relying upon the presence of risk factors for predicting the likelihood of heart attacks and strokes. That is, the current standard of care simply does not do enough to actually prevent disease. The Bale/Doneen Method advocates optimal care for every individual patient identified as needing cardiovascular disease treatment.

The disease treatment paradigm is founded on the discovery of whether plaque is present in the arterial bed. Preamble

Medical practice presents the potential for situations in which the interests of the physician and the patient do not coincide. Ethical principles have always served to guide physician behavior and to protect patients’ interests and wellbeing. Every practice model is liable to present ethical challenges, and all physicians, regardless of the way that they are compensated, are bound to follow the same professional ethical principles.

Novel medical practice models may present previously unconsidered ethical challenges. These challenges call for new guidelines to clarify ethical physician behavior and to protect patients. Novel medical practice models may also serve to uncover new ways to align patient and physician interests and to deliver care with fewer potential ethical pitfalls than existing models.

In its mission to support and promote innovation in medical practices for the betterment of patient care, the AAPP affirms the following ethical principles for all physicians.

Statement of Ethical Principles

  1. A physician shall act in accordance with the AMA Principles of Medical Ethics.
  2. A physician in a retainer practice shall act in accordance with the AMA Ethical Policy for Retainer Practices.
  3. A physician shall be keenly aware of potential conflicts of interest which result from receiving third party remuneration, whether from insurers, pharmaceutical companies, employers, the state or federal government, or research grants. Physicians should make these potential conflicts known to their patients whenever they may affect the ability of the physician to act purely in the patient’s best interest.
  4. A physician shall strive to develop practices that align physician financial incentives with patient interests.
  5. Providing uncompensated care, except in an emergency, is not ethically mandatory. However, uncompensated care of the indigent is profoundly praiseworthy. We invite every physician to volunteer to provide such care in an amount that is in accordance with that physician’s highest calling to perform charitable acts.

Matthew J. Priddy, MD - President

Dr. Priddy is board certified in family medicine and is proud to serve as President for the American Academy of Private Physicians (AAPP), where he has held a seat on the Board of Directors since 2012. He completed his training at the IU School of Medicine and his residency at St. Vincent Hospital in Indianapolis, serving as Chief Resident his final year. A second-generation physician, Dr. Priddy is passionate about the positive outcomes empowered by personalized medicine and the degree of improved patient service that is offered by direct care practice models. He and his wife, Dr. Jen Priddy, live in Carmel and enjoy good food, travel, supporting the local arts, and entertaining friends.

Susan M. Bavisotto - Member

Susan Bavisotto is the Patient Care Coordinator, Medical Assistant, Director of Training and Implementation for Access Healthcare Direct, located in Apex, NC. Susan has helped practices in 17 states learn how to optimize a DPC practice and has helped train their staff on the many day to day logistics within a practice as well as software implementation. Susan also serves as the executive director for the nonprofit DPCMH association.

Tom Blue - Member

Tom Blue has extensive experience developing innovative delivery models for primary care, preventive medicine, employer-sponsored wellness programs, and related technologies. He continues to drive the exploration and delivery of new innovations to the n1Health network of physicians.

Tom is recognized as an authority and a frequent speaker on the topics of concierge and fee-for-service medicine as well as employee health risk management. He has appeared on Fox Morning News, ABC News, CNET Live Radio, The New York Times, Industry Standard, Wired and numerous other programs and publications.

Tom Blue joined the AAPP in 2009 as the Executive Director, now the Chief Strategic Officer on the board of the AAPP. He brings a passion and vision for the industry that has brought momentum and expansion to the organization.

Mr. Blue believes this is an exciting time for the AAPP with the explosive growth in the field of private medicine. “We see private medicine as the “tip of the spear” – leading the way for benefits of new technology and innovation to reach patients. Brian Forrest M.D. - Member

Dr. Forrest completed medical school at UNC Chapel Hill and his residency in Family Medicine at Wake Forest University. He also completed a Holderness Medical Research Fellowship in Diabetic Hypertension and an Albert Schweitzer Fellowship in Public Health while at UNC Chapel Hill. He founded Access Healthcare P.A., Access Wellness, L.L.C., Innovadoc (now DBA Physician Care Direct) and Access Healthcare Direct. He is a pioneer in Direct Primary Care with his original clinic operating in this model for its twelfth year. He has helped physicians around the country to transition to new models of care such as the Access Healthcare Model. He teaches a class for UNC Chapel Hill School of Medicine called “New Models of Care” and also “Disruptive Innovation in Health Care” Practicum for the NCSU Poole College of Management MBA program. He was a featured presenter at “12 Gurus,” a TED type presentation live on Broadway at New World Stages. He has made numerous TV and radio appearances and has been featured on the cover of Medical Economics and the Weekly Standard Magazine for his practice innovation. Currently, he is consulting with physicians in multiple states to transition to the Direct Care Model and manages a national network of similar practices. His clinical focus is on improving outcomes in cardiovascular disease. Currently, he serves on the Board of Directors for COSEHC. He is an amateur magician and teaches martial arts in the discipline of Hap Ki Do. Dr. Forrest was named as one of the Triangle Business Journal’s 40 under 40 and currently serves as Past President on the Board of Directors of the NC Academy of Family Physicians. He is an Adjunct Associate Professor in the Department of Family Medicine at UNC Chapel Hill and ECU and a Master Preceptor for the BCBSNC Family Medicine Scholars Program. He was recently appointed to the AAFP Commission on Quality and Practice.

David C. Rosenberg, M.D. - Member

David Rosenberg, M.D., is a board certified Family Physician. He received his medical degree from the University of Miami in 1988 and completed his residency in Family Medicine at The Washington Hospital in Washington, Pennsylvania in 1991. After practicing Emergency Medicine at Palm Beach Gardens Medical Center for two years, he started his private practice in Jupiter, in 1993.

Jordan Shlain M.D. - Vice President

Dr. Jordan Shlain is a practicing primary care doctor, a digital health entrepreneur, a writer, rancontuer and respected thought leader in national health policy. He is known in the health community as an advocate for physicians and patients.

Dr. Shlain sits on the board of the bi-partisan Washington, D.C. Hope Street Group, and is a Mayoral appointed Commissioner on the Health Service Systems Board of San Francisco. He is an advisor to the Aspen Institute and serves on the board of the American Academy of Private Physicians, WildAid and the Institute for Responsible Nutrition’s Doctors Food Project.

After graduating from UC Berkeley and before attending Georgetown Medical school, Dr. Shlain spent a year teaching in Western Kenya with Harvard affiliated WorldTeach.

He is an active health investor and advises many venture capital and private equity firms. He is the founder and chairman of Heathloop, a Silicon Valley company dedicated to proactively engaging doctors and patients between visits around meaningful outcomes.

The Compliance Committee is the objective body of the AAPP that reviews and evaluates compliance issues/concerns within the medical industry as it relates to Private Medicine. It’s evaluation of rules, policies and regulations of regulatory agencies are being accurately maintained, organized and disseminated; helping the AAPP members and their practices maintain the highest standards in ethics and compliance.


  • Education Committee - the Education Committee will identify, establishes and guide association strategies in the development of new education programs and course delivery in association with the two annual AAPP conferences.


  • Governmental Affairs Committee - the Government Affairs Committee will serves as the voice of the AAPP on Capitol Hill, in the 50 state capitals, and within regulatory agencies to advance the interests of the private physician and medical industry.


  • Technology Committee - the Technology Committee will assess emerging technology, and work with AAPP membership to vet this new technology for integration in to the private medicine industry.


  • Compliance Committee - will identify, support, and promote the highest legal compliance requirements and guidelines established by state and federal governmental entities and encourage the highest standards in ethical and responsible practices in private medicine.

What is a private physician?


Since the mid 1990s, private physicians and their practices have been recognized by many names in the press: concierge physicians, boutique practices, direct care practices, and retainer practices.
What they all have in common is a private (direct) financial relationship with their patients and a commitment to restoring the doctor-patient relationship by eliminating the practice’s financial dependence on insurance and Medicare reimbursements.


Why is this important to you?

Much of what you probably hate about he health care system, your doctor hates just as much. Wait times are too long. Appointments are too short. And medical decisions that should be made by you and your doctor are often too heavily influenced by decisions made by payers. When medical practices rely solely on insurance reimbursements for their survival, the payers inevitably define and shape the care you receive. This is true even if you don’t have health insurance.


Your choice.

No one wants to pay more for health care, but a growing number of people are realizing every day that an investment in a relationship with a private physician is a smart decision. The private medical movement gives you the ability to make your own decision out the type of care you want for yourself and your family.

  • Maybe you’re happy with the care, but you don’t want to wait so long to receive it.
  • Maybe you have trouble leaving work or home for medical care and wish you could work with your doctor more by phone and email – or even get a house call.
  • You may have a high deductible or no insurance at all and you just want to pay a clearly defined, fair cash price for your care.
  • Or you may want a medical relationship that provides time and technology focused prevention and wellness.

You will find these possibilities and more by exploring a relationship with a private physician. Each practice and physician personality is unique, so like you do for other services, you may want to shop and compare before making your decision.

How To Choose Your Doctor


As you will discover, there are many types of private physician practices to choose from. As you shop for the one that’s right for you, here are a few things for you to keep in mind.

There is a practice that suits almost any budget regardless of whether you have insurance. You may think that private medicine is only for wealthy people. This is NOT true. The average fee of a private physician is between $135 and $150 per month. And MANY are under $100/month. Some practices are designed to serve the needs of wealthy people and executives. Many are designed specifically to provide great care for regular people who may be uninsured or have high deductible insurance. Don’t cheat yourself of a great relationship with a physician because you think you can’t afford it.

It’s ok to shop.

The last time you started a new relationship with a doctor, you probably just showed up one day for an appointment and hoped for the best. This isn’t how it has to work in private medicine. Private physicians recognize that your relationship is important. To help you make your decision, they generally invite you to ask questions, see the practice and meet the doctor BEFORE you decide to use the practice. So don’t wait until you’re sick to search for your private physician.

Think in advance about what you want.

This may sound obvious, but in health care, we have gotten used to a one-size-fits-all experience. In private medicine, this is no longer true. You may want a doctor who happily uses email or will conduct an appointment by Skype when there is no medical need for you to come to the office. Maybe you want a practice that is heavily focused on wellness, nutrition, and disease prevention. Maybe you have a lot of doctors and want one who will coordinate the work of all of the others. Know what you’re looking for, and you will likely find it. Understand how the practice works with insurance.

Many private physician practices accept your insurance and collect a retainer or membership fee for services that are not covered by your insurance. Others have chosen not to work at all with insurance. If you have insurance, sometimes these practices will help you collect from your insurance company yourself. Different approaches work well for different people. Just be sure you understand how the practice works to help you make your decision.

Don’t be afraid to ask.

You have likely gotten used to rushing through every experience with a medical practice. Now is your chance to slow down and ask your questions. If a practice tells you they focus on wellness. Ask what that means. Ask how they are different than other practices?

If convenience is most important to you, ask about wait times. Ask how many patients the physician will manage. Most private physicians manage 50% to 90% FEWER patients than other doctors. This smaller number of patients enables the practice to attend to you more closely. Ask about these numbers.
